Community Organisation Training

We offer training to our community partners on a range of interventions relating to Smoking Cessation, Pharmacotherapies and Behaviour Change.

Refer someone

One You Cheshire East is part of a national health initiative supported by Cheshire East Council, along with Reed Wellbeing.

One You Cheshire East provides an integrated and easy-to-access option for residents to address a range of lifestyle issues.

You are able to refer suitable patients to a range of services including Weight Management, Smoking Cessation, Physical Activity, Falls Prevention, Family Health and Maternal Health.

Be Smoke Free

Patients can access free community and specialist stop smoking services.

Their Health Coach will tell them about nicotine replacement products and other stop smoking medicines. They can also recommend which product or combination of products could work for them.

Move More

Patients can access free appointments with our Health Coaches.

They will assess your current activity levels and suggest appropriate ways they can move more. They will set goals and help them to review their progress.

They can also try our free group exercise classes.

Lose Weight

 

We offer free courses to help people to achieve and maintain a healthy weight.

Our weight management course starts with a 20 minute assessment to discover the best way for the patient to lose weight.

They then will have access to a range of courses, including free use of the Slimming World programme.

 

Family Weight Management

Free programmes are available to families wishing to lose weight and improve physical activity.

We offer free one-to-one workshops with a Health Coach and access to an exercise session every week.

Our support is personalised to their age and needs and includes advice and guidance for parents and guardians.

When you refer someone for this programme, please provide the parent or carer’s details for email address, phone number and home address.

Please note that to refer a child, the child must be above the 91st percentile for BMI to be eligible for referral.

 

Stand Strong

If you have concerns about a patient at risk of falling we offer a free programme to help.

We offer free Otago strength and balance training programmes to help people remain mobile and independent.

The patient must live in Cheshire East, be aged 65 years or over and have either poor balance or mobility or has fallen in the past 12 months.

Healthy Baby & You

We offer free support to pregnant mothers who would like to stay active, quit smoking and manage their weight throughout their pregnancy.

If they have given birth in the last 12 months they can also join our free weight management course.
